Adults: 8-16 mg depending on indication. Chemotherapy: 8 mg 1-2h prior, then 8 mg BID for 5 days. Radiotherapy: 8 mg 1-2h before, then 8 mg q8h. Post-op: 16 mg 1h prior to anesthesia.
