What is TEVA-TERIPARATIDE INJECTION used for?

TEVA-TERIPARATIDE INJECTION is indicated for the treatment of postmenopausal women with severe osteoporosis who are at high risk of fracture or who have failed or are intolerant to previous osteoporosis therapy. It is also indicated to increase bone mass in men with primary or hypogonadal severe osteoporosis who have failed or are intolerant to previous osteoporosis therapy. The effects of teriparatide on risk for fracture in men have not been demonstrated. Additionally, it is indicated for the treatment of osteoporosis associated with sustained systemic glucocorticoid therapy in men and women who are at increased risk for fracture. Severe osteoporosis may be confirmed by low bone mass or the presence or history of osteoporotic fracture. Non-vertebral fractures may manifest as back pain, height loss, or kyphosis.

How is it administered?

TEVA-TERIPARATIDE INJECTION should be administered as a subcutaneous injection into the thigh or abdominal wall. The recommended dosage is 20 mcg once a day. The maximum lifetime exposure to TEVA-TERIPARATIDE INJECTION for an individual patient should generally be 24 months. Use beyond 24 months should only be considered if the patient remains at or has returned to having a high risk for fracture. The initial administration should occur under circumstances where the patient can sit or lie down, in case of orthostatic hypotension. Patients should receive supplemental Calcium and vitamin D if dietary intake is inadequate. TEVA-TERIPARATIDE INJECTION is a clear and colourless solution; do not use if solid particles appear or if the solution is cloudy or coloured. Do not use past the stated expiration date. No data are available on the safety or efficacy of intravenous or intramuscular injection of teriparatide. Patients and caregivers should receive proper training on pen use and read the Pen User Manual. The 2.4 mL prefilled pen is not primed before each dose and can be used for up to 28 days if stored properly. After 28 days, the pen should be discarded even if it contains unused solution. The pen should never be shared.

Contraindications

TEVA-TERIPARATIDE INJECTION is contraindicated for: Hypersensitivity to teriparatide or any of its excipients. Pre-existing hypercalcemia. Severe renal impairment. Metabolic bone diseases other than primary osteoporosis (including hyperparathyroidism and Paget's disease of the bone). Unexplained elevations of alkaline phosphatase. Prior external beam or implant radiation therapy involving the skeleton. Bone metastases or a history of skeletal malignancies. Pregnancy and nursing mothers. Pediatric patients or young adults with open epiphyses.

Warnings and precautions

Carcinogenicity: Two carcinogenicity bioassays in Fischer 344 rats showed that teriparatide injections resulted in increases in bone tumours, including osteosarcoma, in a dose- and duration-dependent manner. Osteosarcoma was not observed in teriparatide clinical trials, but has been reported in a post-market setting. However, observational studies have not shown an increased incidence of osteosarcoma with teriparatide treatment. TEVA-TERIPARATIDE INJECTION should not be prescribed to patients at increased baseline risk for osteosarcoma (e.g., Paget's disease, pediatric and young adult with open epiphyses, history of radiation therapy involving the skeleton). General: The maximum lifetime exposure to TEVA-TERIPARATIDE INJECTION for an individual patient should generally be 24 months. Use beyond 24 months should only be considered if the patient remains at or has returned to having a high risk for fracture. Urolithiasis: The frequency of urolithiasis was similar in clinical trials between teriparatide and placebo. However, teriparatide has not been studied in patients with active urolithiasis. Caution should be exercised in patients with active or recent urolithiasis or pre-existing hypercalciuria due to the potential to exacerbate this condition. Hypotension: Isolated episodes of transient orthostatic hypotension were observed in short-term clinical studies, typically within 4 hours of dosing and resolving within minutes to hours. Patients experiencing symptoms should not drive or operate machinery until asymptomatic. Hypercalcemia: Although symptomatic hypercalcemia was not observed in clinical trials, patients should contact a healthcare provider if they develop persistent symptoms of hypercalcemia (i.e., nausea, vomiting, constipation, lethargy, muscle weakness).
