Adults: 0.6 mg/kg IV for intubation; infusion 2.5-4.3 mg/min. Infants/small children: 2 mg/kg IV. Older children: 1 mg/kg IV. IM: up to 3-4 mg/kg (max 150 mg) if IV inaccessible.
