What is VANCOMYCIN HYDROCHLORIDE FOR INJECTION, USP used for?
VANCOMYCIN HYDROCHLORIDE FOR INJECTION, USP is indicated in the therapy of severe or life-threatening staphylococcal infections in patients who cannot receive or who have failed to respond to the penicillins or cephalosporins or who have infections with staphylococci resistant to other antibiotics, including methicillin. It should be used only to treat infections that are proven or strongly suspected to be caused by susceptible bacteria. Vancomycin has been used successfully alone in staphylococcal endocarditis, and its effectiveness has been documented in other staphylococcal infections including osteomyelitis, pneumonia, septicemia and soft-tissue infections. Oral vancomycin should be used for the treatment of staphylococcal enterocolitis and antibiotic-associated pseudomembranous colitis produced by Clostridium difficile.

How is it administered?
Administer at a rate of no more than 10 mg/minute or over at least 60 minutes. Adults: 500 mg every 6 hours or 1 g every 12 hours. Adjust dose for renal impairment.

Rapid bolus may cause hypotension/shock. Ototoxicity can occur at high serum levels (>80 mcg/mL). Nephrotoxicity risk increased with high concentrations or prolonged treatment.
