How is it administered?
CKD: Starting dose 0.45 mcg/kg IV or SC weekly. Cancer: 500 mcg SC every 3 weeks or 2.25 mcg/kg SC weekly. Titrate to maintain Hb 100-115 g/L (not to exceed 120 g/L).

Contraindications
Uncontrolled hypertension; Pure Red Cell Aplasia (PRCA) following ESA treatment; sensitivity to mammalian cell-derived products; hypersensitivity to the drug or any ingredient.
Warnings and precautions
Increased risk of death, serious cardiovascular events, and stroke when Hb >130 g/L. Risk of tumor progression in cancer patients. Risk of PRCA and seizures.
