How is it administered?
Administered by SC injection (except hemodialysis). Prophylaxis: 2,850-5,700 IU daily. Treatment of DVT: 171 IU/kg once daily. Unstable angina: IV bolus 86 IU/kg then SC 86 IU/kg every 12 hours.

Contraindications
Hypersensitivity to nadroparin or heparin; history of heparin-induced thrombocytopenia; acute infective endocarditis; active bleeding; severe renal insufficiency (CrCl < 30 mL/min) for treatment doses.

NEVER administer IM. Risk of spinal/epidural hematoma with neuraxial anesthesia. Risk of hemorrhage increases with renal failure. Monitor platelet counts. Latex allergy potential.
