How is it administered?
Rescue: 10 mg/m2 IV/IM every 6h until MTX < 5 x 10-8 M. Colorectal cancer: 200 mg/m2 slow IV injection before 5-FU. Max IV rate: 160 mg leucovorin per minute.

How is it stored and handled?
Vial contents diluted with saline/Ringer's stable 24h at room temp; with D5W/D10W stable 12h; with D10W in saline stable 6h. Unused solution should be discarded.

Contraindications
Pernicious anemia or other B12 deficiency megaloblastic anemias; known hypersensitivity to any ingredient; intrathecal administration.
Warnings and precautions
Do not administer intrathecally (fatal). Severe/fatal GI toxicity (mucositis, diarrhea) and myelosuppression when used with 5-FU. Monitor for SJS/TEN. Enhances 5-FU toxicity.
