15-20 mg/kg/day (up to 4 g/day in severe infections) via continuous or intermittent IV infusion. IV bolus/push is unacceptable. Replace with oral form as soon as possible.

Hypersensitivity to macrolides; concurrent astemizole, terfenadine, cisapride, pimozide, ergotamine; history of QT prolongation/arrhythmia; electrolyte disturbances; use with lovastatin/simvastatin.

Serious risk of QT prolongation and Torsades de Pointes. Clostridium difficile-associated disease (CDAD). Infantile hypertrophic pyloric stenosis (IHPS) in infants. Hepatic dysfunction.
