How is it administered?
Induction: 40-50 mg/kg IV (initial loading) or 1-5 mg/kg/day orally. Maintenance: 1-5 mg/kg orally daily, 10-15 mg/kg IV every 7-10 days, or 3-5 mg/kg IV twice weekly. Adjust for bone marrow compromise.
