How is it administered?
Subcutaneous injection. Prevention: 3,500-4,500 IU or 50-75 IU/kg daily. Treatment of DVT/PE: 175 IU/kg once daily (max 18,000 IU/day). Dialysis: 4,500 IU bolus.
