How is it administered?
EpiPen and EpiPen Jr are intended for intramuscular administration into the anterolateral thigh, not the buttock. It is designed to inject through clothing or directly through the skin. The effective dose is usually in the range 0.005-0.01 mg/kg bodyweight. Adults: The usual dose is 0.3 mg for intramuscular use. Children and adolescents over 30 kg weight: The usual dose is 0.3 mg for intramuscular use. Children between 15 kg and 30 kg in weight: The usual dose is 0.15 mg for intramuscular use. Children between 7.5 kg and < 15 kg in weight: The suitability of EpiPen Jr has to be judged individually. Children below 7.5 kg in weight: The use is not recommended unless in a life-threatening situation and under medical advice. It is recommended that epinephrine be given at the start of any reaction. In the absence of clinical improvement or if deterioration occurs after the initial treatment, a second injection with an additional EpiPen or EpiPen Jr auto-injector may be necessary. The repeated injection may be administered 5 – 15 minutes after the first injection. All individuals receiving emergency epinephrine must be immediately transported to hospital for evaluation and observation.

There are no absolute contraindications to the use of epinephrine in a life-threatening allergic situation.
Warnings and precautions
EpiPen and EpiPen Jr are for immediate administration as emergency supportive therapy only and not a substitute for immediate medical care. Patients should seek immediate medical or hospital care after administration. All patients prescribed EpiPen/EpiPen Jr should be thoroughly instructed on its use. Patients with a history of anaphylaxis are at risk for subsequent episodes and death, and should always carry injectable epinephrine. Biphasic anaphylaxis (recurrence of symptoms 1 to 48 hours later) is possible. In patients with a thick subcutaneous fat layer, there is a risk of epinephrine not reaching muscle tissue, leading to suboptimal effect. Antihistamines and asthma medications are not first-line treatments for anaphylaxis. Adverse reactions may occur in patients with hyperthyroidism, phaeochromocytoma, severe renal impairment, prostate adenoma, hypercalcaemia, hypokalaemia, elderly patients, and pregnant women. Epinephrine should ONLY be injected into the anterolateral aspect of the thigh. Do not inject intravenously, into the buttock (risk of Clostridial infections/gas gangrene), or into digits, hands, or feet (risk of blood flow loss). Hold the leg firmly during injection in young children to prevent lacerations, bent needles, and embedded needles. Serious skin and soft tissue infections, including necrotizing fasciitis and myonecrosis caused by Clostridia, have been reported at the injection site. Seek medical care for signs of infection. The presence of certain conditions (cardiac arrhythmias, coronary artery or organic heart disease, hypertension, diabetes, Parkinson's disease, narrow-angle glaucoma, asthma) is not a contraindication in acute, life-threatening situations, but caution is advised.
